This form is to be used only by Tribe Lacrosse Parents Requesting a Refund


Before completing this form, please read our Refund Policy.



AGREEMENT:

As parents or guardians of my child, I have read the Tribe Lacrosse refund policy, and am aware that after team formation, or March 1 (Spring) / September 17th (Fall Ball), whichever occurs sooner, for any Clinics or Special Events I am not eligible for a full refund of registration fees, or that after Midnight on the scheduled start day of a clinic, I am no longer eligible for any refund of fees.


I understand that I must return all equipment to Tribe Lacrosse prior to consideration of this request and that any delays in this process will affect the refund amount pursuant to the Tribe Lacrosse policy.
